| When Ana arrived at our hospital, doctors and nurses had waited her for a long time and everything they need had been prepared. After a short rest, Ana and her husband and her brother were pleasurable to attend a birthday party which was held for Ms Lin (a recovered patient with gastric cancer from Malaysia). Pro Xu, the president of our hospital, Pro Ye and Dr Niu (the two Vice-president of our hospital) and other doctors and nurses discussed her conditions carefully and designed the suitable treatment plan for her after she did necessary examinations. After doing “transarterial intervention therapy” successfully two times, her conditions became better than before. However, maybe their culture background is a far cry from ours. The staff of our hospital must spend 3 times, more efforts than that given to other patients in order to soothe Ana. We always asked some help from her family via Email. Through our attempt from several sides, Ana’s emotion became better. And we had to change the arranged treatment a little. After evaluating on Ana's acceptability & mental condition, we decided to add safety measures and perform some treatments over two or three separate sessions each. From 15:20 to 16:40 on 4 Jan 2005, Dr Niu and his assistants did Cryosurgery Successfully for Ana.
The tumors are localized by untrasound. To avoid direct puncture of the tumor and to try to have some noncancerous tissue interposed between entry point and the tumor tissue. An 18-gauge needle is placed in the tumor center, the needle position is confirmed in both imaging planes. A J-shaped guide wire is positioned in the tumor through the needle. A sheathed dilator (3-8mm) is introduced into the tumor over the guide wire. The dilator is removed, and the sheath is left in place. Cryogenic probes are then placed into tumor through the sheath. The sheath is then pulled back. Cryogenic material is circulated through the probes. The iceball created by this treatment is visualized and monitored by real-time untrasound, as the leading edge of the iceball echogenic. The tumors are frozen at maximum flow rate for 15 minutes, and are thawed for 5 minutes, and then refrozen for another 10 minutes. The cryogenic probes are turned on maximum flow rate for an additional 15 minutes, thawed for 5 minutes, and then refrozen for 10 minutes. Dr Niu said they also did “Brachytherapy: Iodine seeds implantation” for Ana at the same time of the operation. Through the process of the whole treatment she can talk with us freely. Now her condition right now is stable. In order to keep safe for her, our doctors let her live in ICU until about 24 hours later. Best wishes for Ana!
